Our Team -- Ready and Able to Fill Every Need

Because we have an extensive array of resources, we are able to accommodate nearly every patient's and family's needs.  Through coordinated, interdisciplinary care, we ensure that all members of the team consistently provide top-quality care.  From making sure initial referrals are handled expeditiously, to keeping the lines of communication open between physicians, our medical director and RNs, we involve every member of the team and keep everyone up to date on the patient's and family's status.

Our services include:

  • Medical direction for patient care, coordinated between the hospice medical director and the patient's attending physician
  • Home visits by registered nurses and licensed practical nurses, to assess, monitor and treat the patient’s condition
  • Pain and symptom management
  • Education and assistance so family members can be involved in patient care 
  • If desired, chaplain services for the patients and family members
  • Hospice aide and homemaker services to address the patient’s personal needs
  • Bereavement counseling to guide the patient and family through the grieving process
  • Counseling, assessment and related services where appropriate and/or requested 
  • Medical supplies and equipment to aid in patient care 
  • Physical, speech, and occupational therapy, when appropriate
  • Dietary counseling when appropriate
  • Support of trained volunteers to assist the patient and family members with day-to-day tasks and needs such as laundry, cooking, errands, etc.
